He swore an oath to save lives, but some cancers require an immediate, lethal excision.

The Incision
The megaphone shrieked, slicing through the crisp autumn canopy of the city park. On the bandstand, the speaker wore crisp black fatigues, his chest puffed out like a rotten vulture’s, bellowing slogans that churned Mick Combre’s gut.
Mick didn’t blink. He knew what hell tasted like. Long before he traded combat boots for surgical clogs, he had lived through firefights in scorched deserts and swampy valleys the world never put on the evening news. He had buried brothers-in-arms who died stomping out this exact poison.
Now, Mick spent his mornings in white scrubs, fingers coaxing faltering pulses back from the brink of oblivion. But he knew pathology when he saw it. Malignancy didn’t negotiate; it choked healthy marrow until nothing remained. The only way to stop a cancer was radical excision. Rake it out at the root.
He watched the speaker wrap up, tossing insults into the small, jeering crowd before descending the wooden steps alone toward the wooded western trail. Mick tracked the man’s stride, the rhythm of his breathing, the soft jugular notch visible above the collar. Precision. Tidy, silent execution. Just like a delicate bypass, every cut had to be calculated before the first incision.
Mick fell into step twenty yards behind, slipping his gloved hand into the heavy wool pocket of his overcoat. His fingers wrapped around cold steel. The trail bent sharply into the shadow of the pines, miles from any witness. Mick closed the distance until he could hear the gravel crunching beneath the man’s combat boots. Ten feet. Six feet. The man stopped, suddenly sensing the breath on the back of his neck, and turned.
Mick’s hand emerged from his coat.
The knife never decides its own purpose—the surgeon does. Does Mick strike to destroy a monster, or does the oath he swore to heal pull his hand back from the dark? Tell me: how does this confrontation end?
